Medicaid Medical Eligibility Determination for Long Term Care

Individuals seeking Medicaid Long Term Care services must meet specified medical and financial eligibility requirements. Medical eligibility is determined through an application and assessment process administered by the Bureau of Elderly and Adult Services (BEAS) in accordance with medical criteria established by law. Financial eligibility is determined by the Division of Family Assistance (DFA) in accordance with defined criteria for income and resources specific to the Medicaid Long Term Care. The Bureau of Elderly and Adult Services and the Division of Family Assistance work together to determine ultimate eligibility decisions.

What is needed to make a referral for application

The Medicaid Referral Application (2 pages) for medical eligibility determination may be completed electronically in a MS Word format for submission by e-mail or printed in an Adobe Acrobat format and completed by hand for fax submission.  If 276 A&B Patient Care Referral forms are sent to BEAS, they will be returned to the referring party with instructions for accessing and submitting the new forms.

Where to apply

To apply for a Long Term Care medical eligibility determination, contact the regional NH ServiceLink Resource Centers throughout the state. Updates to points of contact will be adjusted and posted on this website as they occur.

How eligibility will be determined

Clinical determination of Nursing Facility Level of Care and eligibility for waiver participation will be determined by an in-person assessment by a registered nurse employed by the state or county government. Nurses will use a comprehensive, objective assessment instrument approved by the Legislative Health and Human Services Oversight Committee. MED assessments will be scheduled by the nurse shortly after the financial eligibility application has been initiated with the Division of Family Assistance. Assessments will be conducted at the applicant's current location. The assessment process will be the same for both Nursing Facility and HCBC-ECI applications.

Additional information

Questions pertaining to these changes or the application process, or requests for further information can be submitted by email to LTC@dhhs.state.nh.us. Voice messages can be left by phoning 271-0549.  BEAS will make every attempt to provide timely response to all inquiries.  Updates to any of these processes will be posted on this website as they occur.
